Referrals for an examination must be initiated by a physician or an appropriate agent of the physician (e.g., nurse practitioner, physician’s assistant). As a result, your physician’s office will typically contact us directly to schedule your appointment.
On occasion, your physician may instead ask you to call us and make an appointment, in which case you should call our scheduling department between 7:30am and 5:30pm at (425) 486-8868 or 1-800-282-6516 (Toll free in Washington State).
Our schedulers will help you select from one of our 12 laboratory locations and offer you further instructions to prepare for your exam.
Please have the following information available:
- Patient’s full name
- Date of birth
- Patient’s phone number
- Type of exam
- Indications (symptoms)
- Name of insurance
- Referring physician’s first and last name
- Referring physician’s phone number
- Referring physician’s fax number
If the exam requires fasting, we will do our best to schedule a morning exam.
