• Name of insurance
• Referring physician’s first and last name
• Referring physician’s phone number
• Referring physician’s fax number

If the exam requires fasting, we will do our best to schedule a morning exam.

Cancellation & No Show Policy: Any patient that fails to show or cancels/reschedules an appointment and has not contacted our office with at least 24 hours’ notice will be considered a “No Show” and charged a $50 fee
